Position Description:
1. Assesses and evaluates new patients at intake into program. Interviews patient/family and reviews medical history. Completes initial assessment: including subjective and objective data, functional skill levels and other appropriate information. Completes additional specialized testing including documentation based on patients individualized needs.

2. Performs goal setting during the initial assessment and throughout the program, and participates in discharge planning. Establishes functional goals, short term and long term, in conjunction with patient. Identifies probable discharge needs including assistance, equipment and follow-up therapy needs at initial assessment, making adjustments throughout the course of care. Reviews patient's goal and documents in initial assessment. Documents patient progress including tolerance in daily flowsheet notes. Documentation includes home exercise program initially and reviewed as appropriate. Participates in multidisciplinary meetings as appropriate. Completes discharge instructions and education and documents appropriately.

3. Assists in determining progression of therapy program. Advances patient's interventions / activities as appropriate based on long term goals and patient condition. Follow-through for plan and appropriate changes to plan of care are acknowledged in patient progress notes on the appropriate basis.

4. Conducts therapy session in a safe professional manner. Instructs and supervises patients on safe and appropriate therapy routines, including exercise routines as appropriate. Safely implements appropriate therapy modalities as indicated. Instructs and monitors functional activities, including swallowing, communication and/or speech training as appropriate, as indicated in the treatment program. Alerts appropriate medical professional(s) to problems with therapy sessions, responds appropriately and makes adaptations as needed. Conducts patient/family/caregiver training and education.

5. Maintains current knowledge/certification/licensure. Pursues professional growth and development. Maintains current state license and ASHA certification. Maintains current CPR certification. Participates in inservice programs, outside CE courses, special projects and other assignments. Attends annual mandatory Georgetown Healthcare System inservices and documents in continuing education record.

6. Provides patient and family education in appropriate areas. Initiates and completes patient / family education related to expectations, rights and responsibility in treatment program. Completes patient / family education related to diagnosis(s), home program, and follow through programs§ Assists with patient groups, classes, newsletters, etc as means to educate patients and families.

7. Provides direction to those assisting/learning the therapy role; i.e., new employees, students. Orients new staff and students to the speech therapist role. When precepting a student, provides input into his/her evaluation.

8. Attends all required Safety Training programs and can describe his/her responsibilities related to general safety, department/service safety and specific job related hazards. All required safety education programs have been attended. Provides complete and accurate responses to safety questions. Operates assigned equipment and performs all procedures in a safe manner as instructed. Maintains work area and equipment in condition required by department standards. Demonstrates proper body mechanics in all functions. If identified leader, provides for the safety of department employees.

9. Follows the GHS Exposure Control Plans/Bloodborne and Airborne Pathogens. Demonstrates knowledge of techniques, procedures and correct use of protective barrier equipment. Ensures a safe environment by instituting appropriate control measures. Completes annual education programs.

10. Demonstrates respect and regard for the dignity of all patients, families, visitors and fellow employees to ensure a professional, responsible and courteous environment. North Austin Medical Center – Pharmacy
The pharmacy provides 24 hour inpatient pharmacy services for the patients of North Austin Medical Center from the main pharmacy located on the ground floor. ICU services are provided from the 1st floor ICU satellite pharmacy. The NAMC Pharmacy was one of the first in Central Texas to offer fully bar-coded medications that are linked with the facilities electronic medication administration record system (eMAR) and one of the first to offer electronic provider order entry (ePOM) capabilities. The NAMC Pharmacy is able to provide most all pharmaceutical needs including sterile products, chemotherapy, compounding, educational and clinical pharmacy services. The pharmacy currently provides rotation opportunities for pharmacy interns in pharmacy management, adult medicine, and institutional practice. Most rotations are associated with the University of Texas. The NAMC Pharmacy also provides rotation opportunities for pharmacy technicians who are seeking certification. Most rotations are associated with Austin Community College.

Position Description:

The clinical Exercise Physiologist is responsible for professional care and related assistance to patients in all phases of Cardiac Rehabilitation. The Exercise Physiologist will provide direct and indirect patient care in both the inpatient and outpatient setting. The Exercise Physiologist will assess, evaluate and prescribe safe and effective exercise for patients recovering from a heart attack, heart surgery or who have been diagnosed with cardiac disease. Labor and Delivery Unit Description:

South Austin Hospital's Women's Services department is divided into 4 areas L&D, Postpartum, Level I Nursery and, Level II Nursery. We have a 24-bed post partum/gyn surgical unit, which consists of post partum mothers, GYN surgical patients, and occasional med/surg overflow. Our Level I Nursery is equipped for 16 newborns. 3 bed Level II Nursery. South Austin Hospital's Labor and Delivery unit consists of 7 LDR's, 2 Labor Rooms, 3 Ante partum rooms, 3 bed recovery rooms, two c-section suites, and 1 GYN operative suite. We currently have an approximate delivery rate of 2000 births per year.

Our Women's Services nurses are very involved with patient education and safety. The nurses have adopted a new improved infant security system to increase the security and safety of our patients and their newborns. The staff has developed patient education booklets to supplement the “Newborn Channel”. They have started lactation services for new parents and are very involved with educating prior to discharge. Women's Services has also developed new orientation manuals for the new staff at SAH. They are committed to training and educating all staff on board.

Position Description:

The Registered Nurse (RN) utilizes the nursing process to direct and deliver comprehensive nursing care to patients on an assigned unit in a safe and proficient manner according to individual patient needs. Provides direct patient care based upon South Austin Medical Center Hospital Standards of Care and Standards of Practice, Polices, Procedures and Practice Guidelines. Provides guidance and supervision to Licensed Vocational Nurses (LVN), Nursing Technicians, Nursing Assistants and Unit Secretaries. He/she acts as patient and advocate with the Hospital environment. Assumes Charge Nurse responsibilities and accepts assignments on other units when necessary.

*1) Administers care for the obstetrical patient having surgical procedure in the Labor & Delivery Operating Suite. Intraoperative and post anesthesia care is in accordance with Hospital established standards of care.

*2) Performs steam sterilization processing of instruments and supplies for the MCH unit in accordance with Hospital standards for sterilization. Assures sterilization parameters have been met before utilizing supplies.

*3) Assists with the care of the newborn at deliveries. Maintains skills in neonatal resuscitation.

*4) Demonstrates assessment skills in both high risk and low risk obstetrical patients. Recognition of maternal and/or fetal problems are identified and reported to the physician.

*5) Assists with the transfer of newborn to another facility when indicated. Notifies charge nurse and House Supervisor of anticipated transfer.

*6) Notifies all surgical team members of emergency and scheduled surgical procedures.

*7) Accurately identifies and bands newborn, mother and significant others with matching identabands. Documents band identification number appropriately in medical record. Verifies band and medical record document with nursery upon arrival to nursery.

*8) Provides emotional support and assists in contracting Pregnancy Loss Support Group for parents/significant others experiencing grief due to newborn condition at birth, parental separation from newborn, neonatal death or fetal demise.

*9) Demonstrates knowledge and provides support to natural mother, agencies, and foster or adoptive parents when newborn is for adoption.

*10) Demonstrates knowledge of individual role in Hospital infant abduction plan.

*11) Demonstrate understanding of HCA's and “Patients First” safety initiative by strict compliance to all safety protocols and procedures.

Department Description:
The Intensive Care Unit is a 16-bed unit. This team-oriented, collegial unit enjoys meeting the needs of complex, critically ill patients. The technology is state of the art and includes CVVH, IABP and hypothermic protocol. Patients in this unit have a variety of cardiovascular, neurological, medical and surgical diagnoses like post open heart surgery, respiratory failure, renal disease, carotid stents, craniotomies, endovascular coiling, sepsis, and other multisystem problems. Caring for the whole family is important to this unit.

The Telemetry Unit is a 25-bed unit. Unit Description:

The Medical/Surgical Department is family centered and has a small-town feel. It is a 36-bed unit. The patient population consists of adult and geriatric patients including general medical, general surgical (appendectomy, cholectomy, colon resections, hernia repairs), orthopedics, oncology, end-of-life care, urology, and pediatrics. Diagnoses include status post abdominal, orthopedic, thyroid and gynecological surgeries.

Our Shared Leadership Council has been very successful in developing changes to education and practice issues in the department. We are committed to the success of our peers and the care provided to our patients. There are many changes expected in our future one of which is a transition to a laptop cart for documentation and medication administration via electronic medication administration record (e-MAR).

The unit provides a set schedule that repeats every six weeks. The nursing personnel work 12-hour shifts and work every third weekend. There are ancillary staffs of Unit Secretary and Certified Nursing Assistances.

3. Assists in determining progression of therapy program. Advances patient's interventions / activities as appropriate based on long term goals and patient condition. 4. Conducts therapy session in a safe professional manner. Instructs and supervises patients on safe and appropriate therapy routines, including exercise routines as appropriate. Safely implements appropriate therapy modalities as indicated. Instructs and monitors functional activities, including swallowing, communication and/or speech training as appropriate, as indicated in the treatment program. Alerts appropriate medical professional(s) to problems with therapy sessions, responds appropriately and makes adaptations as needed. Conducts patient/family/caregiver training and education.

5. Maintains current knowledge/certification/licensure. Pursues professional growth and development. Maintains current state license and ASHA certification. Maintains current CPR certification. Participates in inservice programs, outside CE courses, special projects and other assignments. PositionSummary:
The Lactation Consultant provides current, manageable and meaningful lactation information and support for patients in compliance with hospital standards. The Lactation Consultant functions within the philosophy, policies and procedures of the hospital and represents the hospital within and outside the organization. Functions include: maternal/infant dyad breastfeeding support, education and intervention in both inpatient and outpatient settings; telephone consultations; and lactation education for other healthcare providers. The Lactation Consultant works as a team member with other healthcare disciplines to achieve optimal patient care outcomes.

Essential Job Responsibilities:

1. Employee's conduct must reflect HCA's values and commitment to the Code of Ethics and Compliance Program.

2. Provides outpatient education and support for expectant and new parents.

3. Provides inpatient education and support for new parents.

a. Performs daily rounds on breastfeeding mothers to assess learning needs.

b. Provides breastfeeding assistance and instruction daily as indicated.

c. Provides individual breastfeeding education and assistance to postpartum patients and patients whose infants are in NICU.

d. Documents patient's behavior, responses and progress on Lactation Progress Notes.

e. Responds promptly and courteously to all incoming calls within 24-48 hours of their receipt.

f. Serves as a resource person to staff as appropriate.

g. Responsible for maintaining current educational and informational materials on a daily basis.

4. Maintains open lines of communication with office personnel and other healthcare members.

a. Attends 85% of regularly scheduled individual and departmental meetings.

b. Relates information to other healthcare team members to ensure continuity of patient care as appropriate.

c. Orients new employees of the Women's Pavilion to lactation services as appropriate.

d. Provides continuing education/training for nursing staff on lactation during their orientation and on an annual basis, including Ed Express.

e. Consults with Director of Women's Pavilion regarding problems and/or medical issues as needed.

f. Provides telephone follow-up calls to breastfeeding patients after discharge.

g. Maintains and provides statistics on breastfeeding rates.

5. Exhibits professional behavior.

a. Attends all facility and position appropriate mandatory programs and in-services.

b. Writes a yearly self-evaluation including personal and professional goals.

c. Maintains international board certification (IBCLC).

d. Participates in a minimum of 25 hours of clinical update annually.
e. Demonstrates flexibility in staffing to provide lactation services seven days per week.

Knowledge, Skills and Abilities
Must be able to demonstrate understanding of national patient safety initiatives and be in strict compliance to all safety protocols and procedures as required by both HCA and St. David's Georgetown Hospital.

The Lactation Consultant works autonomously, sets priorities and makes independent decisions of a complex nature. The Lactation Consultant shall demonstrate a capacity for compassion and empathy. The Lactation Consultant interfaces directly with the public, physicians, staff, patients, administration and ancillary/support staff and departments. The Lactation Consultant maintains confidentiality of patient's medical records in compliance with HIPAA. Regular and predictable attendance is required.

A new Lactation Consultant will see patients with a mentor for a period deemed appropriate to qualifications prior to seeing patients unsupported.

Education and Experience:
Required:
Texas Department of State Health Services' Level I and Level II Lactation Management Courses or equivalent. IBCLC (Internationally Board Certified Lactation Consultant).
Minimum of 2 years Maternal and Child Health or lactation consulting experience.
CERPs (Continuing Education Recognition Points): 75 CERPs required for five year re-certification (50 LCERPS specific to Lactation, 20 RCERPS related to lactation, 5 ECERPS related to ethics).
Ten year re-certification by exam is required.

Position Summary:
Responsible for daily function of drug distribution systems, inventory control, drug information and education, departmental performance improvement and medication use activities.

Position Description:
1. Complies with hospital/departmental policies and procedures, i.e., attendance, mandatory education, safety, infection control, etc.

2. Assists in coordinating and supervising departmental functions.
• Takes responsibility to see that required daily functions are completed when working as “MAIN” pharmacist (drug purchases, drug charges, floorstock replenishment, etc.).
• Supervises the pharmacy support staff in a consistent, fair manner and leads by a professional example.
• Effectively communicates important issues with oncoming shift.

• Maintains departmental adherence to state and federal law and professional standards.

3. Accurately and effectively provides medication therapy.

• Appropriately reviews physician order(s) and inputs into the pharmacy profile system.
• Reviews drug regimen for drug allergies, rational therapy, contradictions, reasonable dose and route, reasonable directions, duplication of therapy, drug-drug interactions, other interactions, and adverse drug reactions.
• Applies knowledge of pharmacotherapy to identify patient and family requirements relative to age-specific needs (see age-specific criteria).
• Communicates with other disciplines and the patient to resolve issues.
• Appropriately check all medications prior to dispensing.

4. Actively participates in intervention, performance improvement and professional programs.

3.4 No more than 2 dispensing errors of commission (original order) per month.
3.5 Ensure accuracy of medication order dose instructions, par level, label comments, and container labels.

3.6 Supervise all delegated acts related to drug distribution by supportive personnel.

3.7 Run daily computer reports and MARS.

3.8 Maintain appropriate pre-packing and compounding records.

3.9 Enter IV orders for labels, prepare, deliver, and check profiling and preparation of sterile products on a daily basis.

3.10 Follow procedural guidelines for preparation of all sterile products.

4.0 Provide for control and distribution of controlled substances.

4.1 Adhere to procedural guidelines 100% for CSAR log and control drug receipt.

4.2 Maintain perpetual inventory of C II drugs and conducts monthly audits as assigned.

5.0 Interprets, prepares, and dispenses outpatient med orders, and maintains appropriate records.
5.1 Maintain outpatient prescription files per State Board regulations.
5.2 Document reconciliation of ER take home inventory weekly as assigned.

6.0 Demonstrates ability to select, utilize supplies and equipment in a cost effective manner, and to reduce waste.
6.1 Ensure contract items are ordered through online order entry system.

6.2 Follow procedures for reconciling Accunet exceptions list.

6.3 Transmit wholesale order daily as scheduled.

7.0 Demonstrates an ongoing awareness of; and participates in departmental and hospital performance improvement plans.
7.1 Participates on PI teams as assigned.
7.2 Reviews MAR variances, reconciles discrepancies, and clearly documents reasons for variances on Pharmacy copy of MAR.

7.3 Conducts analysis and summarizes results as assigned.

8.0 Serves as a drug information resource and provides clinical pharmacy services to hospital staff and patients.

8.1 Conducts patient education counseling as requested by physician, other healthcare providers, or based on professional judgement.

8.2 Conducts inservices/orientations for nursing and medical staffs as assigned.

8.3 Documents all clinical interventions and identified prescribing deficiencies in pharmacy computer module

Education and Experience:
Required:
Bachelor of Science in Pharmacy from an accredited college or university
Two years hospital pharmacy experience

Preferred: PharmD from an accredited college or university

Unit Description:

The Medical/Surgical Department is family centered and has a small-town feel. It is a 36-bed unit. The patient population consists of adult and geriatric patients including general medical, general surgical (appendectomy, cholectomy, colon resections, hernia repairs), orthopedics, oncology, end-of-life care, urology, and pediatrics. Diagnoses include status post abdominal, orthopedic, thyroid and gynecological surgeries.

Our Shared Leadership Council has been very successful in developing changes to education and practice issues in the department. We are committed to the success of our peers and the care provided to our patients. There are many changes expected in our future one of which is a transition to a laptop cart for documentation and medication administration via electronic medication administration record (e-MAR).

The unit provides a set schedule that repeats every six weeks. The nursing personnel work 12-hour shifts and work every third weekend. There are ancillary staffs of Unit Secretary and Certified Nursing Assistances.

Required: Graduation from an NLN school of professional nursing.
Preferred: Completion of or actively pursuing a BSN degree. Experience as a Staff Nurse in an acute ICU setting. Required: Must have 2 years of current experience to work PRN.

Position Description:
Physical Therapists assess, evaluated and treat movement dysfunction and pain resulting from injury, disease, disability, or other health related conditions.

The Physical Therapist will assess patient's progress during and following treatment. Evaluation and treatment shall be initiated only by written prescription or confirmed verbal orders of a qualified physician. Therapists will interact with members of this department as well as other hospital departments to provide for essential department operational tasks, including patient treatment and continuing education, infection control, and quality assurance. Physical Therapists will maintain communication through documentation and verbal consultation with nursing staff and physicians to provide optimal patient care. Physical Therapist will work in any or all area of PT including rehabilitation, outpatient and skilled nursing unit (SNF)

This position is an Acute Care Hospital position within a dynamic interdisciplinary department serving orthopedic, neurologic and general medical-surgical needs. Unit Description:

RRMC has a 12 bed state of the art ICU expanding to 16 beds January 2008. Offering excellent compassionate care to a wide variety of patients ranging from complex medical, surgical, pulmonary, neuro, and post cardiac bypass procedures to list a few. We will be expanding to an 18-bed unit in 2008 to include providing post operative care for Neurosurgical patients.

The staff at Round Rock Medical Center's ICU Unit provides care not only to the patient, but also to the family as a whole. St. David's Medical Center has a 65 bed level III NICU, the largest level III NICU in Central Texas. In addition to conventional ventilation, we have oscillator, high frequency and nitric oxide capabilities. We use a body cooling system that induces hypothermia to reduce brain damage. We have the only cerebral function monitoring system in the city . Our transport team is available 24/7 for any hospital requesting a transfer to our unit. POSITON SUMMARY:

The normal work hours for this position are day shift, 2nd shift, and/or 3rd shift; including weekends and holidays.

ESSENTIAL JOB RESONSIBILITIES:

1. Complies with hospital/departmental policies and procedures, i.e., attendance, mandatory education, safety, infection control, etc.

2. Assists in coordinating and supervising departmental functions.

· Takes responsibility to see that required daily functions are completed when working as “MAIN” pharmacist (drug purchases, drug charges, floorstock replenishment, etc.).

Unit Description:
This unit has 22 LDRP rooms, 2 outpatient triage/testing beds and 2 operating rooms for c-sections and postpartum tubal ligations.

The unit is a level I facility providing care to low-risk patients. The LDRP set up enables patients to labor, delivery, recover and postpartum all in the same room and mother and baby have continuous rooming-in after delivery unless complications arise.

We provide 1:1 nursing during labor and 1:4 couplet care after delivery resulting in excellent patient satisfaction.

There is a Level I nursery; however the babies are primarily rooming-in. The Medium Risk Nursery is available for stabilizing sick or premature babies for transport. That nursery can also support babies needing brief (less than 24 hours) additional support like oxyhood and IV therapy. There is one nursery nurse assigned each shift who attends deliveries and monitors any babies periodically kept in the nursery.

This unit is experiencing tremendous growth. There has been a 30% increase in deliveries over the past year bringing us to an average of 120 deliveries a month, with an average of 14 combined inpatient and outpatient encounters each day. Six OB and 2 Family Practice doctors deliver here and there are five pediatricians. A pediatric nurse practitioner is in-house weekdays, three anesthesiologists, and three CRNA's provide 24 hour anesthesia availability. Lactation consultants are on the unit seven days a week as well.

The majority of the staff are registered nurses capable of working L&D, postpartum and newborn nursery. These RNs need at least one year of L&D experience. They must also hold certifications in BCLS, ACLS, and NRP. Completion of the AWHONN Fetal Monitoring Course and the STABLE Course are required but can be completed after hire. A small number of LVNs and RNs without L&D experience work strictly with mother/baby couplets. These nurses also must hold certifications in BCLS and NRP and complete the STABLE Course.

The position of Registered Nurse is a professional position that requires education and experience necessary to make independent decisions for patient care within the scope of the Definition of Nursing Care and Limits of Licensure in the State of Texas. Unit Description:

The Medical/Surgical Department is family centered and has a small-town feel. It is a 36-bed unit. The patient population consists of adult and geriatric patients including general medical, general surgical (appendectomy, cholectomy, colon resections, hernia repairs), orthopedics, oncology, end-of-life care, urology, and pediatrics. Diagnoses include status post abdominal, orthopedic, thyroid and gynecological surgeries.

Our Shared Leadership Council has been very successful in developing changes to education and practice issues in the department. We are committed to the success of our peers and the care provided to our patients. There are many changes expected in our future one of which is a transition to a laptop cart for documentation and medication administration via electronic medication administration record (e-MAR).

The unit provides a set schedule that repeats every six weeks. The nursing personnel work 12-hour shifts and work every third weekend. There are ancillary staffs of Unit Secretary and Certified Nursing Assistances.

Skills/Requirements

EDUCATION/EXPERIENCE:
Required: Graduation from an NLN school of professional nursing.
Preferred: Completion of or actively pursuing a BSN degree. Experience as a Staff Nurse in an acute ICU setting. Required: Must have 2 years of current experience to work PRN.

Labor and Delivery Unit Description:

This area is currently experiencing rapid growth with the monthly delivery rate exceeding 250 deliveries. It includes 13 LDR's, 2 triage rooms, 2 bed recovery rooms for Cesarean Sections and other obstetrical related procedures. This area also has the unique feature of doing newborn transition in the mother's labor room, thus decreasing the separation time between mother and newborn to almost zero. If there is any need for separation, it rarely exceeds 30 minutes in the transition nursery. All staff are NRP, AWHONN-ELECTRONIC FETAL MONITORING certified. An educational push is being done to encourage RNC specialty certification, ACLS, and limited ultrasound training. Focus for education in the future will be for high risk OB. Nursing procedures encourage nursing qualifications to perform placement of fetal scalp electrodes, intrauterine pressure catheters, and for nursing anatomy.

Position Description:

Responsible for professional nursing care and related assistance to patients with a broad range of illnesses.

The RN is involved in direct and indirect patient care. The RN provides direction to licensed vocational nurses (LVNs) and other non-licensed nursing personnel in providing direct patient care. The staff nurse supports the medical care of patients under the direction of the Medical Staff pursuant to the objectives of the hospital. Labor and Delivery Unit Description:

South Austin Hospital's Women's Services department is divided into 4 areas L&D, Postpartum, Level I Nursery and, Level II Nursery. We have a 24-bed post partum/gyn surgical unit, which consists of post partum mothers, GYN surgical patients, and occasional med/surg overflow. Our Level I Nursery is equipped for 16 newborns. 3 bed Level II Nursery. South Austin Hospital's Labor and Delivery unit consists of 7 LDR's, 2 Labor Rooms, 3 Ante partum rooms, 3 bed recovery rooms, two c-section suites, and 1 GYN operative suite. We currently have an approximate delivery rate of 2000 births per year.

Our Women's Services nurses are very involved with patient education and safety. The nurses have adopted a new improved infant security system to increase the security and safety of our patients and their newborns. The staff has developed patient education booklets to supplement the “Newborn Channel”. They have started lactation services for new parents and are very involved with educating prior to discharge. Women's Services has also developed new orientation manuals for the new staff at SAH. They are committed to training and educating all staff on board.

Position Description:

*4) Demonstrates assessment skills in both high risk and low risk obstetrical patients. Recognition of maternal and/or fetal problems are identified and reported to the physician.

*5) Assists with the transfer of newborn to another facility when indicated. Notifies charge nurse and House Supervisor of anticipated transfer.

*6) Notifies all surgical team members of emergency and scheduled surgical procedures.

We use a body cooling system that induces hypothermia to reduce brain damage. We have the only cerebral function monitoring system in the city . Our transport team is available 24/7 for any hospital requesting a transfer to our unit. The all RN staff are NRP (Neonatal Resuscitation Program) certified and many are NCC (National Certification Credentialing) certified in their area of specialty. There is a Neonatologist in house 24/7.

Physical therapy includes:(1) performing and interpreting tests and measurements to assess pathophysiological, pathomechanical, elecrophysiological, ergonomic, and developmental dificits of bodily systmes to determin diagnosis,, tretment prognosis, and preventiom,(2) Planinning, administeringm, and modifying therapeutic interventions that focus on posture, locomotion, strength endurance, cardiopulmonary function, balance, coordination, joint mobiltiy, flexibility, pain, healing, and repair, and functional abilities in daily living skills, (3) providing consultative, educational, research and other advisory services. The Physical Therapist will assess patient's progress during and following treatment. Evaluation and treatment shall be initaited only by written prescription or confirmed verbal orders of a qualified physician. Therapists will interact with members of this department as well as other hospital departments to provide for essential department operational tasks, including patient treatment and continuing education, infection control, and quality assurance. Physical Therapists will maintain communication through documentation and verbal consultation with nursing staff and physicaians to rpovide optimal patient care. Physical Therapist will work in any or all area of PT including rehabilition, outpatient and skilled nursing unit (SNF)

Unit Description:
This unit has 22 LDRP rooms, 2 outpatient triage/testing beds and 2 operating rooms for c-sections and postpartum tubal ligations.

The unit is a level I facility providing care to low-risk patients. The LDRP set up enables patients to labor, delivery, recover and postpartum all in the same room and mother and baby have continuous rooming-in after delivery unless complications arise.

We provide 1:1 nursing during labor and 1:4 couplet care after delivery resulting in excellent patient satisfaction.

There is a Level I nursery; however the babies are primarily rooming-in. The Medium Risk Nursery is available for stabilizing sick or premature babies for transport. That nursery can also support babies needing brief (less than 24 hours) additional support like oxyhood and IV therapy. There is one nursery nurse assigned each shift who attends deliveries and monitors any babies periodically kept in the nursery.

This unit is experiencing tremendous growth. There has been a 30% increase in deliveries over the past year bringing us to an average of 120 deliveries a month, with an average of 14 combined inpatient and outpatient encounters each day. Six OB and 2 Family Practice doctors deliver here and there are five pediatricians. A pediatric nurse practitioner is in-house weekdays, three anesthesiologists, and three CRNA's provide 24 hour anesthesia availability. Lactation consultants are on the unit seven days a week as well.

The majority of the staff are registered nurses capable of working L&D, postpartum and newborn nursery. These RNs need at least one year of L&D experience. They must also hold certifications in BCLS, ACLS, and NRP. Completion of the AWHONN Fetal Monitoring Course and the STABLE Course are required but can be completed after hire. A small number of LVNs and RNs without L&D experience work strictly with mother/baby couplets. These nurses also must hold certifications in BCLS and NRP and complete the STABLE Course.

Position Description:
The Unit Secretary communicates patient information, manually and electronically, to appropriate health care providers. Prioritizes the flow of patient information to all areas.

Position Summary:
Responsible for daily function of drug distribution systems, inventory control, drug information and education, departmental performance improvement and medication use activities. This is a PRN opportunity, on an on call as needed basis.

1.0 Assures customer/personal safety by maintaining a safe and therapeutic environment and by adhering to Employee Health and Safety program.
1.1 Utilizes safety guidelines, material safety data sheets (MSDS), and reports hazards, accidents, and unsafe equipment and/or practices.
1.2 Demonstrates adherence to universal precautions and aseptic technique.

1.3 Verbalizes and/or demonstrates roles and duties in fire, evacuation, and disaster drills.

1.4 Regularly reviews hospital information and/or attends educational activities related to safety and injury prevention.

2.0 Maintains and increases standards of performance and education.

2.1 Takes initiative to increase job knowledge through additional training, cross training skills development, and/to continuing education.

2.2 Attends or fulfills requirements for all mandatory hospital inservices to include annual education, safety update and training by anniversary date.

2.3 Reviews all new and revised policies and procedures or designated manuals.

2.4 Attends at least 80% of department meetings and/or verifies acknowledgement by reading meeting minutes.

3.0 Ensures accurate and efficient drug distribution services.

3.1 Verify new orders against profile for allergies, interactions, duplication and ensure patient weights and allergies are documented on profile.

3.2 Orders are processed within 1 hour of receipt or before next administration time.

3.3 No more than 1 dispensing error of commission per month.

3.4 No more than 2 dispensing errors of commission (original order) per month.
3.5 Ensure accuracy of medication order dose instructions, par level, label comments, and container labels.

3.6 Supervise all delegated acts related to drug distribution by supportive personnel.

3.7 Run daily computer reports and MARS.

3.8 Maintain appropriate pre-packing and compounding records.

3.9 Enter IV orders for labels, prepare, deliver, and check profiling and preparation of sterile products on a daily basis.

3.10 Follow procedural guidelines for preparation of all sterile products.

4.0 Provide for control and distribution of controlled substances.

4.1 Adhere to procedural guidelines 100% for CSAR log and control drug receipt.

4.2 Maintain perpetual inventory of C II drugs and conducts monthly audits as assigned.

5.0 Interprets, prepares, and dispenses outpatient med orders, and maintains appropriate records.
5.1 Maintain outpatient prescription files per State Board regulations.
5.2 Document reconciliation of ER take home inventory weekly as assigned.

6.0 Demonstrates ability to select, utilize supplies and equipment in a cost effective manner, and to reduce waste.
6.1 Ensure contract items are ordered through online order entry system.

6.2 Follow procedures for reconciling Accunet exceptions list.

6.3 Transmit wholesale order daily as scheduled.

7.0 Demonstrates an ongoing awareness of; and participates in departmental and hospital performance improvement plans.
7.1 Participates on PI teams as assigned.
7.2 Reviews MAR variances, reconciles discrepancies, and clearly documents reasons for variances on Pharmacy copy of MAR.

7.3 Conducts analysis and summarizes results as assigned.

8.0 Serves as a drug information resource and provides clinical pharmacy services to hospital staff and patients.

8.1 Conducts patient education counseling as requested by physician, other healthcare providers, or based on professional judgement.

8.2 Conducts inservices/orientations for nursing and medical staffs as assigned.

8.3 Documents all clinical interventions and identified prescribing deficiencies in pharmacy computer module

Skills/Requirements

Education and Experience:
Required:
Bachelor of Science in Pharmacy from an accredited college or university
Two years hospital pharmacy experience

Preferred: PharmD from an accredited college or university

Knowledge, Skills, and Abilities:
Ability to communicate clearly and effectively; excellent oral and written communication skills; ability to relate to diverse populations in an open, friendly, and accepting manner; ability to make timely and sound decisions under stressful conditions. Management of conflicting priorities; problem solving, team building; comfort level with ambiguity and autonomy; ability to form effective working relationships to achieve established goals and objectives. Computer/data entry skills. Adherence to customer service behavioral standards. Must be able to identify medication use safety issues and intervene in professional manner with nursing and medical staffs. Unit Description:

RRMC has a 12 bed state of the art ICU expanding to 16 beds January 2008. Offering excellent compassionate care to a wide variety of patients ranging from complex medical, surgical, pulmonary, neuro, and post cardiac bypass procedures to list a few. We will be expanding to an 18-bed unit in 2008 to include providing post operative care for Neurosurgical patients.

The staff at Round Rock Medical Center's ICU Unit provides care not only to the patient, but also to the family as a whole. Visiting hours are open to allow support and convenience for patients and families. Currently staffed with 6 nurses when full, as well as Tech and/or unit secretary. Nurse to patient ratio 1:2. Self-scheduling is an added convenience for the staff.

Position Description:

Responsible for professional nursing care and related assistance to patients with a broad range of illnesses. The RN is involved in direct and indirect patient care. The RN provides direction to licensed vocational nurses (LVNs) and other non-licensed nursing personnel in providing direct patient care. The staff nurse supports the medical care of patients under the direction of the Medical Staff pursuant to the objectives of the hospital. In collaboration with other health team members, follows the philosophy, policies and goals of the facility and department.

Required: Current licensure in Texas as a RN. Certification in basic cardiopulmonary resuscitation.
